Join our awesome team as we work together to serve out community. Counselors will serve individuals or populations in specific life and social skill areas. QMHP will provide case management/coordination and integration of services. Maintaining documentation pertaining to medical records, completion of progress notes, and service plans of assigned individuals. Completes all duties related in support of developing independent living and social skills. Mental health professional will provide community-based services, crisis de-escalation, social skills training, anger management skills, self-sufficiency and self-management skills, and responsibility and goal setting.
